There are a lot of questions here but an experienced photographer should have no problem answering them for you. If the company has a Frequently Asked Questions brochure or page on their web site, please take a look there first- often times most questions are already answered for you- save yourself and them some time!
Who will be the photographer for our wedding? If not you, can we meet the photographer? Can we see the personal work of the particular photographer?
What happens if the booked photographer cannot shoot on our day (whether no longer working for you or on sick leave?)
How many assistants will help? Do they also photograph?
What attire will you and any assistants wear?
How experienced are you with photography? What kind of education do you have? What kind of real-world experience do you have?
How many years have you been a wedding photographer, and how many weddings have you photographed?
Do you shoot in digital or standard film? Why?
What is your primary shooting style?
Can we give input or direction? Can we give you a shot list to work from?
You should have already seen the photographer's work, but sometimes their portfolio shows different styles. Be sure you know whether you get to choose the style or if the photographer gets to choose.
Can other people take photos while you are taking photos?
What are your criteria for choosing what you would photograph at my wedding?
Do you limit the number of photographs you take?
What time do you arrive to setup your equipment?
Do you attend for certain events or for a certain amount of time? Are there overtime charges if extra time is needed?
Do you bring backup equipment with you?
Do you provide a written contract and guarantee?
What are your policies regarding proofs?
If shooting digital: Do we get all of the high-resolution digital images or a certain number?
If not shooting digital: What are your policies regarding negatives?
